Insurance, TPA Billing & Doctor Commissions in GridX HMS

How GridX HMS handles the money side of a hospital — payer and TPA claims, split billing between patient and insurer, and two-phase accrual accounting for doctor and referral commissions.

Clinical software gets the attention, but it's the financial side that decides whether a hospital is solvent. Two areas trip up most facilities: getting paid by insurers and panels, and paying doctors and referrers correctly. GridX HMS builds both into the same double-entry engine that records every other transaction, so the numbers reconcile themselves.

Billing isn't always to the patient

In a cash-only world, the patient pays and you're done. In the real world, a large share of revenue comes through payers — insurance companies and third-party administrators (TPAs). The Insurance & TPA module models this directly:

  • Maintain a register of payers and the patients covered by each
  • Bill an insured patient against their payer, with the invoice split between the patient share (co-pay) and the insurer share
  • Submit claims and track them through adjudication

Because the split happens on the invoice itself, your receivables are clean: you can see at any time how much is owed by patients versus how much is sitting in claims with each insurer.

Double-entry underneath everything

Every invoice — cash, credit, or insured — posts a balanced journal entry to the general ledger. GridX HMS ships a full accounting engine: chart of accounts, account categories and types, parties (patients, vendors, doctors, employees), journal vouchers, and a per-account ledger view. Invoices, service bookings, and admissions all auto-post, so the books are a by-product of doing the work rather than a separate month-end scramble.

Payments settle into bank accounts you define — each mapped one-to-one to a ledger account — so card, online, and bank receipts land in the right place automatically.

Paying doctors and referrers

Doctors, technicians, and referrers are often paid a share of what they generate, and calculating that by hand is where money leaks. The Commissions & Shifts module lets you define commission rules:

  • Type — a fixed amount or a percentage
  • Entity — a specific doctor, technician, referrer, or employee
  • Scope — a particular item, a particular service, or everything that entity touches

Commissions then auto-calculate on invoices, service bookings, and diagnostic sessions. Tiered/slab structures and TDS/withholding at settlement are supported.

Two-phase accrual accounting

Here's the part that keeps the books honest. By default, GridX HMS uses two-phase, accrual-basis commission accounting:

  1. At billing, the commission liability is accrued — debit Commission Expense, credit Commission Payable. The expense is recognised when it's earned, even though no cash has moved.
  2. At settlement, the payable is cleared — debit Commission Payable, credit Cash/Bank for the net, and credit TDS Payable for any withholding.
  3. If the source transaction is cancelled, the accrual is reversed.

The whole flow is idempotent (guarded by an accrual transaction ID) and best-effort, so it never blocks a patient from being billed. The result is that your Commission Payable always reflects what you genuinely owe — not a number someone reconstructs from a spreadsheet at payout time.

Reporting that ties it together

The reporting module reads from the same ledger: doctor-wise and test-wise share, employee commission summaries and transactions, receivables and payables, vendor payables, and cash-collection statements by date, user, and shift. Everything reconciles because everything was posted from the same source events.
